Are such events perversion or illness?

Today I saw a very strange and disgusting incident on social media; they were saying an elderly man did something harmful to himself in public. As a first reaction, everyone mocked and cursed, but then someone wrote, “at this age, such uncontrolled behaviors can sometimes be a neurological disorder.” Honestly, I’m confused. Of course, such behavior is unacceptable in society, but should we directly interpret every strange and nauseating act as perversion? What if he is genuinely sick?

At first, it naturally disgusts a person, but there is really a possibility of illness for an elderly person. In some neurological conditions, the social brake completely disappears, and the person does not behave like they used to.
