I had lip fillers done at one point too, but I didn't tell my wife. As you said, it's hard to notice, so it never came up. If you're not feeling uncomfortable, I don't think there's any problem in not mentioning it.

Thank you for the comments. I’ll add that I'm actually really scared the filling will be noticeable. My friends said, 'you won't notice when it fits perfectly,' but my spouse is very detail-oriented. That's why maybe it will be harder for them to explain 😅

This is your body and your decision. It's perfectly normal to want to have some control over your own life. Your partner's opinion may be important, of course, but it's not more important than your freedom. Women shouldn't have to make decisions just to please others.
